SASA Accident / Incident Report Forms

Accident / Incident Report Guidelines

St. Albert Soccer Association recommends that the following procedure is used when dealing with an Accident or Incident involving a player or coach.

  1. Stay calm but act swiftly and observe the situation.
    1. Is there danger of further injury?
  2. Listen to what the injured person is saying.
  3. Alert the parent/guardian on site of your concern (concussion, severe sprain or injury, etc).
  4. In the event of serious injury, call emergency services.
  5. Do not move someone with serious/major injuries. Wait for emergency services to arrive.
  6. Deal with the rest of the group and ensure that they are adequately supervised and are safe.
  7. Contact the injured person’s parent/guardian if not on site.
    1. Advise to fill out the ASA Insurance Claim policy # 25345A
      1. All players and coaches are covered through registration fees.
    2. Complete an Accident / Incident Report (below).

Emergency Action Plan

  • Dial 9 for an outside line
  • Dial 9-1-1
  • Give your name and suspected injury
  • Give location and address
    • ST. ALBERT SOCCER ASSOCIATION, 61 RIEL DRIVE, ST. ALBERT
    • ST. ALBERT SOCCER PLAYER DEVELOPMENT CENTER, 19 RENAULT CRESCENT, ST. ALBERT
  • Tell them that someone will meet them
    • At the gate of the south end parking lot - 61 Riel Dr.
    • Outside the front door of 19 Renault Cr.
  • Ask for the estimated time of arrival
  • Report immediately to injury site
  • Inform charge person that an ambulance is on the way
  • Go to the ambulance meeting site
    • Closest hospital:
      • Sturgeon Community Hospital
      • 201 Boudreau Rd.
